This resource has been created by the 340B Prime Vendor Program to assist stakeholders by combining the most recently available 340B-specific Medicaid data with publicly available federal data. Please note that additional requirements or contractual obligations that may be in place from third-party payers are not included in this resource.

Dispensing 340B drugs through contract pharmacy is permitted by state; however, neither party will use drugs purchased under Section 340B to dispense Medicaid prescriptions, unless the covered entity, the contract Pharmacy and the State Medicaid agency have established an arrangement to prevent duplicate discounts. Any such arrangement shall be reported to the OPA, HRSA, by the covered entity.
